    [EN] Nowadays, the use of prostheses for transfemoral amputees is painful, and users have poor control of leg movements. Still, the possibility to use an exoprostheses has a direct impact on the quality of life of amputees. This paper presents the project developed by TEQUIR to design a new concept of prosthesis that enables amputees to use a prosthetic leg, with a more natural and comfortable support. This innovation will have a direct impact on improving the quality of life of transfemoral amputees.[ES] En la actualidad, el uso de prótesis externas para amputados transfemorales es doloroso y los usuarios tienen un control deficiente de los movimientos de la pierna. Aun así, la posibilidad de emplear una exoprótesis tiene una repercusión directa en la calidad de vida de los amputados.     Autonomous cortisol secretion in patients with primary aldosteronism: prevalence and implications on cardiometabolic profile and on surgical outcomes

    Purpose: The aim of this study was to evaluate the prevalence of autonomous cortisol secretion (ACS) in patients with primary aldosteronism (PA) and its implications on cardiometabolic and surgical outcomes. Methods: This is a retrospective multicenter study of PA patients who underwent 1 mg dexamethasone-suppression test (DST) during diagnostic workup in 21 Spanish tertiary hospitals. ACS was defined as a cortisol post-DST >1.8 ÎŒg/dL (confirmed ACS if >5 ÎŒg/dL and possible ACS if 1.8–5 ÎŒg/dL) in the absence of spe cific clinical features of hypercortisolism. The cardiometabolic profile was compared with a control group with ACS without PA (ACS group) matched for age and DST levels. Results: The prevalence of ACS in the global cohort of patients with PA (n = 176) was 29% (ACS–PA; n = 51). Ten patients had confirmed ACS and 41 possible ACS. The cardiometabolic profile of ACS–PA and PA-only patients was simil ar, except for older age and larger tumor size of the adrenal lesion in the ACS–PA group. When comparing the ACS–PA group (n = 51) and the ACS group (n = 78), the prevalence of hypertension (OR 7.7 (2.64–22.32)) and cardiovascular events (OR 5.0 (2.29–11.07)) was higher in ACS–PA patients than in ACS patients. The coexistence of ACS in patien ts with PA did not affect the surgical outcomes, the proportion of biochemical cure and clinical cure being similar between ACS–PA and PA-only groups. Conclusion: Co-secretion of cortisol and aldosterone affects almost one-thi rd of patients with PA. Its occurrence is more frequent in patients with larger tumors and advanced age. However, the cardiometabolic and surgical outcomes of patients with ACS–PA and PA-only are similar

    Revisiting ‘place’ in a realist novel: ‘Thinking space’ in Galdós’s Torquemada en la hoguera (1889)

    Departing from the premise that Galdós’s close engagement with space and place deserves to be at the forefront of scholarly attention, this article provides an in-depth study of their significance in Torquemada en la hoguera. It begins by analysing the relationship between the novel’s locations and the real world, demonstrating that the author codes the city of Madrid to express social concerns and promote reader engagement. It then proceeds to examine the public and private spheres, before highlighting the “place of the imagination” in the novel. It reveals that, as in Galdós’s press articles, reality is used as a springboard in Torquemada en la hoguera and, drawing upon recent theories, it posits that places serve as a framework for engaging readers with contemporary concerns and as an imaginative springboard for Galdós. They trigger what is effectively a “thinking space” for the author and it is through unravelling their significance that we can fully appreciate Galdós’s psychological sensitivity, the novel’s modernity, its symbolic value, and imaginative depth. The article concludes by proposing that Galdós’s works deserve to be re-examined as “Novels of the Geographical Imagination” and urges readers to revisit the significance of space and place therein

    Impacts of retailing attractiveness on freight and shopping trip attraction rates

    International audienceThis paper provides an integrated framework for assessing the relations between retailing attractiveness and both freight and shopping trips generated in urban areas. The proposed framework deploys a trip attraction (FTA) and a shopping trip attraction (STA) model then carries out a retailing attractiveness analysis. First, an overview on trip generation and attractiveness related to retailing is made to motivate and position the present work. The paper introduces then the methodological framework and describes the FTA and STA models as well as the attractiveness indicator proposed. After that, both FTA and STA flows are assessed on Lyon's conurbation. Then, the relations between retailing attractiveness and retailing-based trip attraction rates are analysed on the basis of linear regression analysis. The results show that both FTA and STA rates have a direct relation with retailing attractiveness but the category of urban space needs also to be included in those analyses. Finally, as a conclusion, main implications of the proposed framework are addressed for both researchers and practitioners
